You can unlock the nurse uniform after Katie 100% girfriend progress
clothes.dat edit:
replace the #medic outfit section with this set of line
#medic outfit
SETC medictr torso medictop medic
SETC medictr feet bask1 bask1eris
HIDE medictr legs
you can replace it with any t-shirt or shirt model/clothes on CJ's wardrobe. You can modify and upload the files as you please on another modding sites but shows the original credits or link to this creation.
